Review - ANZCA Training Programme Curriculum





The Australian and New Zealand College of Anaesthetists (ANZCA) is directly responsible for the examination and qualification of anaesthetists, and the standards of anaesthesia practice, in both Australia and New Zealand. As part of the College's current strategy, the ANZCA Training Programme has undergone a major review.
The ANZCA Curriculum Review Project was designed to ensure that the curriculum remains contemporary in terms of both clinical content and educational method. It was also designed to address the pressing issue of a significant anticipated expansion in the numbers of graduating students applying for anaesthesia training.
The review was coordinated by the College's Education Development Unit (EDU) and oversight was provided by the Curriculum Review Working Group (CRWG) reporting to the College’s Education and Training Committee (ETC) and ANZCA Council.

ANZCA Curriculum Review Project
Project Overview
Background
A review of the current ANZCA Training Programme was planned since its introduction in 2004. The Curriculum Review Project was initiated in late 2008 to coincide with the first cohort of ANZCA Trainees completing the full programme.
The review was also undertaken in response to a number of issues facing the medical systems in which Anaesthetists work and train:
- increased number of junior doctors seeking a place in vocational training from 2012, due to increased places offered by medical schools in both Australia and New Zealand
- government calls for expanded training settings (for example, into the private sector and rural areas).
- maintaining a contemporary training programme (both in clinical content and educational method)
Aim
The aim of the Curriculum Review Project is to enhance educational quality in three key areas of the ANZCA Training Programme:
- The teaching, learning and assessment methods used;
- The processes undertaken to manage and enhance the programme, including:
- Continuous evaluation and quality improvement systems;
- Development, training and support for ANZCA Teachers.
- The operational matters associated with the programme.
Consultative Phases
Submissions
The first consultative stage in the Curriculum Review Project was completed in January 2009 and took the form of an open submissions process. Any organisation/instutution, group or individual having an interest in the ANZCA Training Programme was able to comment on the current training programme, indicate desired outcomes of training and also propose innovative ideas for the curriculum. Invitations were also sent to key stakeholders.
The submissions were open from 1 October 2008 to 31 January 2009.

Survey
The second consultative phase in the Curriculum Review Project was completed in October 2009 and took the form of a survey of all ANZCA Fellows and Trainees. The survey focussed on the major issues surrounding the ANZCA Training Programme, as identified in the submissions process and/or by relevant College committees, working groups and operational units.
The survey was open from 21 September until 31 October 2009.

Project Outcomes
The outcomes of the Curriculum Review Project included an ANZCA Curriculum Framework and a set of Recommendations for Curriculum Change.
ANZCA Curriculum Framework
A curriculum framework is an organised set of learning outcomes which define the scope of content to be learned and what needs to be taught and assessed. The ANZCA Curriculum Framework explicitly defines, all aspects of contemporary practice of Fellows of the Australian and New Zealand College of Anaesthetists (FANZCAs) and will be used to plan and deliver all aspects of the ANZCA Training Programme (including its content, teaching, learning and assessment tools).
To develop this framework, the College has adapted an internationally recognised and objectively researched framework used by many medical specialties worldwide; the CanMEDS Curriculum Framework (for further information, see the CanMEDS website). The ANZCA Curriculum Review Working Group (CRWG) coordinated the adaptation of this framework. During the Consultative Phases of the review, input was sought regarding the ANZCA Curriculum Framework from all stakeholders. Further amendments have been included following a review of other relevant curriculum frameworks, as well as input provided by the ANZCA Trainee Committee and ANZCA Regional/National Committees.

Recommendations
Information gathered during the Curriculum Review Project has been used by the Curriculum Review Working Group (CRWG) to develop a set of recommendations for changes to the ANZCA Training Programme.
Sources of information for development of the recommendations were:
- ANZCA Curriculum Review Submissions
- ANZCA Curriculum Review Survey
- Evidence-based (or best-practice) educational principles
- Principles of the ANZCA Curriculum Review Working Group (CRWG)
- College Strategy
- Government requirements/requests
- Medical Council (both Australian and New Zealand) requirements/requests
In collaboration with the ANZCA Assessments Subcommittee (ASC), the CRWG developed an initial draft of the recommendations which then underwent several rounds of review and refinement following further input from the ANZCA Education and Training Committe (ETC) and its subcommittees.

Project Reporting
There were three stages of reporting for the Curriculum Review Project:
- Approval by ETC - March 2010
- Approval by Council - April 2010
- Official Dissemination - May 2010 (ANZCA ASM)
- The outcomes of the Curriculum Review Project were officially disseminated at the 2010 ANZCA Annual Scientific Meeting (ASM), which was held in Christchurch, New Zealand.
- Following dissemination of outcomes from the Curriculum Review Project, a process of re-writing and implementation will need to be undertaken. The ANZCA Curriculum Redevelopment Project will be launched in due course.
Curriculum Review Working Group (CRWG)
To ensure that a relevant agenda, timeline and project framework was set for the Curriculum Review Project, ANZCA Council convened a Curriculum Review Working Group (CRWG).
Roles and Principles
The role of the CRWG was to act as a steering group for the curriculum review process; and specifically to:
- set the agenda, timeline and project framework for the curriculum review process
- provide guidance on direction of the curriculum review process
- provide guidance on the methodological approaches to be utilised
- advise on methods to access key groups for participation in the review process
- provide general expertise on the practice of anaesthesia, when required
- oversee the analysis process from the review
- convene specialist reference groups to assist with interpretation of results
The roles of the CRWG did not include:
- re-writing the curriculum
- representation of any specific anaesthetic specialty/interest content
The principles of the CRWG were:
- that the CRWG should be of a workable size (aiming for 12-15 members)
- that the CRWG should be a primarily internal working group (with community and federal government representation)
- that the CRWG will meet primarily via teleconference
- that members will not be representing any individual specialty expertise area, as specialty content expertise will be sought at a key stage in the review process
- that the CRWG will contain members covering various aspects of experience with the FANZCA curriculum
- that the CRWG will contain members with the required expertise for steering the curriculum review process
- that the CRWG will contain members covering various stake-holder perspectives
- that on completion of the review, which will involve recommendations regarding curriculum change, the CRWG will be disbanded

Recommendations for Curriculum Change - Approved. Following input and approval from the ANZCA Education and Training Committee (ETC), including its subcommittees, the Recommendations for Curriculum Change were approved at the April meeting of the ANZCA Council. These recommendations will be officially disseminated, along with other Project Outcomes, at the 2010 ANZCA ASM.
ANZCA Curriculum Framework - Approved. Following a final round of input from the Regional/National Committees, the ANZCA Curriculum Framework, developed by the CRWG during the Curriculum Review Project, was approved at the April meeting of the ANZCA Council. The framework will be officially disseminated, along with other Project Outcomes, at the 2010 ANZCA ASM.

ANZCA Curriculum Framework - Provisionally Approved. The ANZCA Curriculum Framework, developed by the CRWG during the Curriculum Review Project, was approved at the February meetings of both the ANZCA Education and Training Committee (ETC) and Council. It is anticipated that the framework will undergo a further round of input from Regional/National Committees before being submitted to the April 2010 meeting of Council for final approval. The finalised framework will be officially disseminated, along with other Project Outcomes, at the 2010 ANZCA ASM.

Meeting - Monday 1 February 2010
The fourth meeting of the ANZCA CRWG was a teleconference held from 16:00-18:00 AEDT. The aim of the workshop was to continue drafting the final recommendations from the ANZCA Curriculum Review Project. Members of the CRWG reviewed and discussed the draft ANZCA Curriculum Framework, which has been developed during the review through information provided in the two rounds of consultation (for more information on these consultative phases, see the Submissions and Survey pages). A draft set of recommendations for curriculum change were also reviewed and discussed, with members agreeing to provide detailed amendments electronically. Members agreed that the final ANZCA Curriculum Framework and Recommendations for Curriculum Change documents should be submitted to the February Education and Training Committee (ETC) and then April Council for final approval. The final set of approved outcomes will then be disemminated at the May 2010 ANZCA Annual Scientific Meeting.

Meeting - Monday 23 November 2009
The third meeting of the ANZCA CRWG was a workshop held at ANZCA House. The overall aim of the workshop was to begin drafting the final recommendations from the ANZCA Curriculum Review Project, developing these primarily from the information received during the recent ANZCA Curriculum Review Survey.

Along with members of the ANZCA CRWG, other relevant representatives were in attendance to ensure the production of a comprehensive and coordinated set of recommendations for the ANZCA Training Programme. Members of the ANZCA Assessment Subcommittee (ASC) were invited to ensure that recommendations regarding assessment are well aligned with recommendations regarding other aspects of the curriculum. The ASC membership also includes Chairs of other relevant committees (e.g. Workplace-based Assessment, Primary Exam, Final Exam and the Trainee Committee). The Chair of the ANZCA Clinical Teacher Development Working Group (CTDWG) and Coordinator of the Distance Education Working Group (DEWG) was also in attendance to ensure alignment of recommendations for teacher development and support activities. In addition, some relevant College Unit representatives will be in attendance to provide their perspectives (i.e. Training and Assessments, Finance, IT and Library).
To begin the workshop attendees were given an overview of the Curriculum Review process to-date, including an overview of results from the recent Curriculum Review Survey. The attendees then worked in small groups to review and discuss, in detail, selected sections of the survey results.
Following the workshop the CRWG will continue to refine the recommendations for changes to the ANZCA Training Programme and the ANZCA Curriculum Framework, with a final report due to the Education and Training Committee in February 2010.

Meeting - Friday 06 March 2009
The second meeting of the ANZCA CRWG was a workshop held at ANZCA House and was aimed at intiating development of a discussion document for the Curriculum Review. We were delighted to have expert participation at this workshop by Dr Jason Frank (Associate Director, Office of Education, The Royal College of Physicians and Surgeons of Canada, RCPSC). As one of the leaders in the development and implementation of the CanMEDS Curriculum Framework in both Canada and around the world, Dr Frank’s experience was most valuable to the working group.

To begin the workshop attendees were given an overview of the Curriculum Review process to-date, including an overview of results from the recent Submissions Process. Dr Frank then gave a presentation on the implementation of the CanMEDS Framework, including: teaching, learning and assessments methods; and implementation pitfalls to be avoided. Following this, further development work was conducted by the group on the draft ANZCA Curriculum Framework, by analysing the submission responses regarding desired outcomes of ANZCA Training. Later the working group explored and debated major issues for the Curriculum Review.
Further work in 2009 is expected to involve a census-style survey of Fellows and Trainees and further refinement of the ANZCA Curriculum Framework.

Official Close of Submissions. Submissions for the ANZCA Curriculum Review closed on 31 January 2009.
The ANZCA CRWG was very pleased to receive in excess of 100 submissions from a range of stakeholders, including: ANZCA Fellows, Trainees, Staff, Committees and Working Groups. A diverse range of external stakeholders also made submissions, including: other Colleges (both Australasian and International); anaesthetic associations; various government/regulatory bodies (in both Australia and New Zealand); as well as other affiliated professional groups (such as midwives and operating room nurses); and a number of international experts in medical education.
The ANZCA CRWG would like to thank everyone who coordinated and/or contributed to a submission, your time and effort is very much appreciated and your thoughts and opinions on the ANZCA Training Programme will be invaluable for the next stages of the curriculum review process.

Progress Update - Tuesday 25 November 2008
Submission Deadline Extended. The ANZCA CRWG would like to thank everyone who had already taken the time to make, or begin preparing, a submission. Whilst submissions were encouraged to be made as soon as possible, it was recognised that some groups (particularly external parties) were finding it difficult to prepare a response by the original deadline (31 December 2008). As such, the submission deadline was extended; until 31 January 2009. While submissions received up until 31 January 2009 were included in the review, submissions were reviewed and utilised as they were received. In particular, submissions received prior to the first meeting of the ANZCA Clinical Teacher Development Working group (CTDWG) (11 December 2008) formed an important basis for the review and re-design of the support and development initiatives the College offers to those providing clinical teaching opportunities to ANZCA Trainees.

Meeting - Thursday 14 August 2008
The first meeting of the ANZCA CRWG was a workshop held at ANZCA House, and marked the beginning of this latest review of the ANZCA Training Programme. The President, and members of the College Executive, started the day with an official welcome. Workshop attendees were then given an overview of the curriculum review process by the Education Development Unit (EDU) and began work to agree a framework and set of principles for the process.

The outcomes of the workshop were:

Agreement of the use of the CanMEDS-2005 framework for the revised curriculum (for further information, see the CanMEDS wesbite)
- Development of principles for the review process, in particular the need for:
- alignment of teaching, learning and assessment within the curriculum;
- clear and feasible organisation of the curriculum (especially in regards to selection, clinical experience and progression issues);
- a system of on-going programme evaluation and improvement for the training programme; and
- a system of clinical teacher development and support available to all clinical teachers (i.e. available to all Fellows working with trainees).
- Agreement on the structure for the submissions process.
By all accounts this first meeting of the CRWG was a success. The next step in the Curriculum Review was a submissions process to canvass opinions of all stakeholders (including Fellows, Trainees, other Colleges etc).
