Incident reporting/monitoring

Learning from both adverse events and near misses is essential for improving quality care. Preparing an incident report, considering the reasons why the incident took place and actions that can be taken if the participant is presented with a similar situation in the future, is a valuable form of practice evaluation. Participants could complete this activity through their department, hospital or practice or use a web based anaesthetic incident reporting system, for example; WebAIRS. 


Credits: Two credits per hour 


Evidence: Brief summary of reported event (paragraph), statement from  department or group or entry on WebAIRS.
