CPD portfolio offline
Fellows who wish to participate offline may use a hard copy of the CPD portfolio.
Hard copy portfolio
Fellows may print the hard copy CPD portfolio and record activities manually.
Annual summary form
Offline users can submit an Annual Summary Form to the CPD Unit who will record their activities and send out the relevant statement or certificate.
Replacement and electronic records
If Fellows require replacement pages for their portfolio or wish to keep an electronic record of any of their portfolio pages, they may download the individual sections of the portfolio below.
Statements and certificates
To obtain an Annual Statement of CPD Participation, please submit an annual summary form and a copy of your CPD plan to the CPD Unit.
To obtain a Certificate of Completion of CPD Triennium at the end of your triennium, please ensure that you have submitted an annual summary form for each year of the triennium and a copy of your CPD plan and evaluation to the CPD Unit.
