Safety alerts

Safety alerts are published in each edition of the six-weekly ANZCA E-Newsletter and the quarterly ANZCA Bulletin.

Some are government or hospital announcements and many have been identified by the ECRI Institute. The ECRI Institute is a non-profit organisation that issues alerts derived from four sources: the ECRI International Problem Reporting System, product manufacturers, government agencies, including the US Food and Drug Administration (FDA) and agencies in Australia, Europe and the UK as well as reports from client hospitals.

It is recognised that some alerts may only involve single or small numbers of cases, there is no denominator to provide incidence and there is not always certainty about the regions where the equipment is supplied. This section can only highlight some of the alerts that may be relevant and it is the responsibility of the hospitals to follow up with the manufacturer’s representatives if they have not already been contacted.

If you are aware of a safety issue in your hospital or are interested in quality and safety issues, please contact the Quality and Safety Coordinator
