Hot Case Assessment Form

All trainees applying to sit the Fellowship Examination (in either general or paediatric intensive care medicine) must complete four practice hot (ICU) cases prior to the closing date for the Examination. The Supervisor of Training (or nominee, which may be a member of consultant staff) should arrange for a series of practice hot cases, preferably a program of cases leading up to the exam. The attached form must be submitted to the Faculty office prior to the closing date, and preferably with the application to sit the Examination.
