Trainee Forms

In the course of the Training Program, you will be required to complete and submit information to the College relating to various stages of the program (e.g., applying for examinations, completion of in-training assessments, changing your address). 

The major forms can be downloaded from this page. If you have a printer that can print double-sided copies, please select this feature, otherwise please insure that all sheets are properly secured and/or identified to avoid pages going astray.

Trainee Registration Form (TRF)

The Trainee Registration Form (pdf icon 328 kB) is required for all Trainees who intend to participate in the ANZCA Training Program, including sitting the Primary Examination. There is a standard registration fee for all trainees in Australia, New Zealand and Affiliated Training Regions.


Training Agreement

The Training Agreement (pdf icon 35 kB) outlines the respective roles and responsibilities of the Trainee and the College in the course of the ANZCA Training Program. This document must be submitted at time of registration.


Payment Form (PAY)

The Payment Form (pdf icon 212 kB) is a generic form used to make a number of payments. NOTE: Payments of the Annual Training Fee should be made using the Annual Training Fee Form, as specific information is required in this instance.


Annual Training Fee Form (ATF)

For trainees in Approved Training a 2008 Annual Training Fee Form (pdf icon 302 kB) must be submitted with a fee to be paid by 31 January each year, or if starting in an Approved Training position after 1 March, within three months of commencement of the position. Beginning in 2008, Trainees not in Approved Training must pay a Holding Fee that maintains their registration in the ANZCA Training Program and entitles them to all communications of the College as well as online access privileges.


In-Training Assessment Form (ITA)

In-Training Assessment forms ITA 1 and ITA 2 are the basis by which the College assesses the clinical training and professional attributes of its Trainees. The ITA process provides a mechanism to give feedback to Trainees on a number of critical performance areas and is a vital part of training assessment. There are two formats:

ITA 1 (pdf icon 105 kB) - used for self-evaluation by the Trainee or for providing Trainee feedback from Senior Department Staff.

ITA 2 (pdf icon 104 kB) - used for In-Training assessment of College Trainees to be completed at six-monthly intervals from the start of rotation or, at the end of the rotation if earlier.


Approved Vocational Training Form (AVT)

In order for training time to be accredited, all trainees must fully complete their Approved Vocational Training Form (pdf icon 293 kB) (including signatures of all relevant Supervisors of Training) and return to the College within three months of completion of the year of Training.


Module Completion Form

A Module Completion Form (pdf icon 93 kB) verifying completion of the various modules must be completed and returned to the College by the Trainee as each component of training is completed.


Primary Examination Form (PEX)

The Primary Examination Form (pdf icon 320 kb) must be submitted to the College before the closing date for applications. Applications will not be accepted if the eligibility criteria (including the required documentation) have not been met. So it is important that the Trainee check his/her Trainee Profile to ensure that College records are current and meet the eligibility requirements.


Final Examination Form (FEX)

The Final Examination Form (pdf icon 254 kB) must be submitted to the College before the closing date for applications. Applications will not be accepted if the eligibility criteria (including the required documentation) have not been met. So it is important that the Trainee check his/her Trainee Profile to ensure that College records are current and meet the eligibility requirements.


EMAC/EMST Courses 

Trainees must satisfactorily complete either the Effective Management Anaesthetic Crises (EMAC) course or the Early Management of Severe Trauma (EMST) course, or an equivalent course recognised by Council as part of their Training Program. These courses are provided by external providers, so Trainees are advised to apply early to participate in either of these courses.

Effective Management of Anaesthetic Crises

Effective Management of Anaesthetic Crises (EMAC) is a course intended to provide techniques in the management of anaesthetic emergencies. EMAC targets not only the technical skills required during an emergency, but emphasises the behavioural aspects of managing anaesthetic crises. Please contact individual centres for bookings and enquiries.

Early Management of Severe Trauma Committee

The Early Management of Severe Trauma (EMST) course provides techniques in the management of anaesthetic emergencies. EMST is a two and half day intensive course in the management of injury victims in the first 1-2 hours following, it is adapted from the Advanced Life Support (ATLS) course of the American College of Surgeons, and emphasises life saving and a systematic clinical approach. The registration form can be downloaded or, applicants can register online via the Early Management of Severe Trauma (EMST) Course page on the Royal Australasian College of Surgeons website. 


Fellowship Application Forms

There are three forms that need to be completed when applying for Fellowship by Examination and Training. These vary slightly by jurisdiction.

  • Application for Fellowship by Examination and Training Form (pdf icon 93 kB) (all candidates). This form is submitted to the CEO once all Training Program requirements have been met. This is checked by the Training & Assessments Unit and if the requirements are complete, submitted at the next Council meeting for approval.
  • A Communication Consent Form. This form has two components: (1) a consent to receive periodic mailings from the College, and (ii) a separate consent to be included in an online Register of Fellows (accessible by FANZCA only). Both of these items are optional.
  • A Health Insurance Commission Notification (for Australian citizens and permanent residents only) that grants automatic recognition of your new specialist Fellowship status for Medicare purposes.
