Safer Care Victoria has recently published a framework and toolkit to guide Morbidity and Mortality (M&M) meetings.

Avant has published a case summary – Physician’s failure to communicate with colleague is unsatisfactory –, which highlights the importance of communication between colleagues and its relationship with safety and quality of patient care.

Oesophageal intubations has been published by Sage Journals in Anaesthesia and Intensive Care.

“Oesophageal intubations are more common than may be realised and can potentially cause significant patient harm even if promptly identified and corrected. Reports of morbidity due to unrecognised oesophageal intubation continue to present in coroner and media reports. This analysis of oesophageal intubations reported to webAIRS aims to provide an in-depth analysis of all events in which oesophageal intubation occurred.”
