Safer Care Victoria (SCV) and Serious Adverse Patient Safety Event (SAPSE) reports
SCV has recently published a position statement that recommends SAPSE reports are provided within the Sentinel Event Portal for purposes of transparency and compliance.
Australian Council on Healthcare Standards Root Cause Analysis report writing training
On 10 March 2026, the ACHS is offering a 3-hour workshop for clinicians on how to write quality RCA reports.
Reporting of adverse events for medical devices by healthcare facilities
From 21 March 2026, Australian healthcare facilities have a mandate to report adverse events for medical devices. Information, including guidance and resources, is available on the Therapeutic Goods Administration’s website.
Instruction for searching the TGA’s database of Adverse Events
The TGA has established a webpage that provides instruction on how to refine searches within its AE database.
WebAIRS update
Obesity may increase the perioperative risk of a patient who is planned for surgery and can be exacerbated by associated medical comorbidities.
The webAIRS team is actively undertaking analyses related to the high BMI population as part of their broader initiative to improve patient safety and quality in anaesthesia.
Reviewing and analysing these incidents and disseminating the results with the anaesthesia community will create awareness and aim to reduce the risks associated with these unique challenges.
